Georgia Pediatric Program In – Home Nursing

Georgia Pediatric Program (GAPP) is a Medicaid program that provides services to medically fragile children under the age of 21 in need of skilled nursing care and personal care support in their homes and community. GAPP is not a waiver but state planned services. Families eligible for Medicaid can apply for GAPP and do not need to meet the same level of care as they would in a skilled nursing facility or an institution. The child must have a medical necessary need. What that means: in order to care for the child outside of a skilled facility the services are desperately needed, and without the services the child would be remanded to a skilled nursing facility. So, your child can receive care at home, at no cost to eligible families.

Oasis Superior Health Care Services are an approved GAPP provider. We partner with your child’s doctors and Georgia Medicaid to build a safe, customized plan of care and bring skilled nurses and trained caregivers right to your door.

Who May Qualify for GAPP Services?

Your child may qualify for GAPP if:

  • They are under 21 years old
  • They have ongoing medical or personal care needs
  • They are eligible for Georgia Medicaid or in another Medicaid program such as Care Source or can become eligible

Do we have to be in the hospital to apply?

No. Families can apply for GAPP from the hospital or from home through an approved GAPP agency like Next Step. Hospital discharge planners and physician offices can also help start the process.

How long does it take to get approved?

“Once a complete GAPP packet is submitted, the medical review team generally has up to 30 days to review and make a decision. Times can vary, but we keep you updated along the way.

What happens when my child turns 21?

GAPP ends on the child’s 21st birthday, but other adult Medicaid waiver or support programs may be an option. We’ll help you plan for a safe transition before that date.
